Terms and Conditions

Appointment Cancellation Policy

Dr Teri Johnson requires 48 hours notice (excluding Sundays) for appointment cancellations for both treatments and consultations.

Dr Teri charges a £50 cancellation fee when an appointment is scheduled but the clients does not attend or cancels with less than 48 hours notice.

We understand occasionally problems occur and appointments cannot be kept. However, if this becomes habitual, we will ask for a £50 deposit at the time of booking your subsequent appointments which can be deducted from your treatment, provided you give more than 48hrs notice should you need to cancel or reschedule.

Booking Consultation Appointments

To book a consultation with Dr Teri a £50 consultation fee is required payable at the point of booking. This can be paid via debit/credit card over the phone, by Paypal online or you can pay by cash by calling in to the clinic.

If you are unable to pay this when you book, the appointment slot will be held for you for 24 hours, after which time if the deposit has not been paid, it will be made available to other clients.

If you need to cancel or rearrange your appointment slot, the deposit can be refunded upon request as long as you give us 48 hours notice as above.

Consultations

Please arrive for your appointment at least 10 or 15 minutes in advance of your appointment time, so that the necessary paperwork can be completed before your consultation.

Late arrival may result in reduced consultation time, treatment time or you may have to forfeit the appointment. Should you need to rebook we ask that you pay a further £50 deposit.

The £50 consultation fee can be deducted from the cost of any treatment you subsequently have with Dr Teri provided it is within 6 months of the consultation.

Sculptra Appointments

Sculptra must be mixed 5 to 7 days in advance of a treatment.

For this reason Sculptra treatments require a deposit of 50% of the treatment value at the time of booking or at least 7 days before the appointment.
If less than 48 hrs notice is given to cancel or reschedule, the full deposit will be retained as a cancellation fee, unless the appointment can be rescheduled within the next few days in which case only the £50 cancellation fee will be applied.

Obligation to perform a treatment

Even if you wish to have particular treatment, if Dr Teri believes the interventions she provides are unlikely to deliver the desired outcome or if a treatment will not be in your best interest, she will decline to perform treatment and will discuss with you the other options available.

Complaints

In the unlikely event of a complaint. Please feel free to discuss your concerns and any issues you may have so we can help you with your concerns.
Please ask for our complaints procedure.

Complications

Injectable procedures have well documented complications, although these are rare, Dr Teri will bring these to your attention at the time of your consultation and treatment. They will also be documented on your consent form.

If you are at increased risk this will be brought to your attention before proceeding with any treatment.
It is therefore very important that you update Dr Teri with any changes in your medical history, including recent pregnancy, breast feeding, any surgery, infections, past or future dental work, all recent medication and herbal supplements so she can fully assess your risk and discuss it with you.

Dr Teri will make sure you have access to help and advice 7 days a week in case you have any concerns. It is important you get in touch if you develop any pain, significant unexpected swelling or blistering or anything else unusual that you were not expecting.
Please contact the clinic by phone on 01606 841255 incase of any concerns unless it is out of hours in which case ring 07858 118117 or email middlewichclinic@aol.com. You will be assessed and arrangements made for management and follow up.

Dr Teri attends regular training and updates to make sure she is up to date with the latest guidance on managing complications.
Should you develop any complication Dr Teri will treat it to the best of her ability, knowledge and training. She will also get advice from her colleagues where appropriate, as most complications can be managed conservatively.

Most of the time she will treat you without further cost with no charge for clinic reviews (although you may have to pay for a private prescription at your chemist unless your GP is happy to prescribe for you).

In the very unlikely event of a complication being more serious and needing further specialist treatment, Dr Teri will arrange for you to be seen, assessed and treated in the NHS.  If you prefer to be seen privately Dr Teri can help liaise with private doctors that specialise in such complications if you prefer. This may mean you have to travel a distance to access them and it would be at your own cost. Please note that private medical insurance may not cover complications arising from cosmetic work.
